At your initial D3 Clinic denture consultation in Alexandria, we ask that you arrive 10 minutes early to ensure any necessary paperwork is completed.
You can also download or complete your new patient forms online prior to your appointment. All forms are submitted securely and delivered directly to our clinic to expedite your check-in process.
By being a few minutes early it helps ensure that we’re best able to respect the time of you and your fellow patients. If you find that you’re running behind, please contact our clinic as soon as possible so that other arrangements can be made.

What to Bring With You
We ask that you bring a copy of your medical records, as well as your most recent X-rays, if applicable. This may eliminate the need for us to request updated X-rays or will provide a baseline to compare your new patient imaging against for reference purposes. Be sure to bring your health insurance card as well, so that we can process claims on your behalf. By providing us with your insurance card in advance, we can verify your coverage and provide you with the most accurate financial breakdown of your proposed care plan.
